Carnival License

Applicant Responsability

  • The dates, hours of operation, and location of the carnival.
  • The name and telephone of the individual who can be contacted at any time during the period the carnival will be on site and/or operating.
  • A copy of the carnival operator’s current and valid “License to Operate Amusement Devices” issued by the MA Department of Public Safety.
  • A list of the ride identification numbers, with a complete description, of all State permitted amusements to be operated on the dates requested.
  • A copy of the Insurance Certificate indicating coverages amounts as required by the MA Department of Public Safety:
  • Proof of Workers Compensation Insurance
  • Proof of Public Liability Insurance
  • A description of how electricity and water will be provided and where those sources are located.
  • A site plan drawn to scale showing the placement of amusements, walking paths and vendors.
  • Proof of appropriate permits from other Town Departments. Example: Board of Health, Building Department, Police Department, Fire Department, etc.
  • A copy of your current CORI Policy as required by the MA Department of Public Safety.
