Pfizer COVID-19 Vaccine Clinic to be held at Hanover High School on August 30, 2021

The Hanover Board of Health in collaboration with Brewster Ambulance Service will be holding a vaccination clinic for individuals 12 years and older on August 30, 2021, from 5:00 PM to 7:00 PM, at Hanover High School, 287 Cedar Street. You don't need to schedule an appointment and walk-ins are welcome.

• Pfizer Consent Form for 12-17 Year Old Individuals
• Brewster Ambulance Registration Form

Important Information

• Register above for an appointment time, one slot per person
• 2 doses are required to be considered fully vaccinated.
• Print 2 copies, complete all form fields, and bring the Brewster Registration Form to both vaccine clinics.
• 12-17 year old individuals must be driven by an adult and bring the consent form signed by a parent/guardian.
