Applicant Responsibility
- A Business Certificate issued by the Town Clerk, if an individual.
- An attested copy of Articles of Organization with Amendments, if a corporation.
- Federal and State Tax Certificates
- Workers Compensation Affidavit
- A detailed plan or sketch, drawn to scale, showing the interior and exterior of the site.
- For Axe Throwing Only - a copy of your Safety Plan.
