Medicare Enrollment Information
We are pleased to offer two great options for your Medicare coverage, Medex 2 with Blue MedicareRx Prescription Drug Plan and the Medicare PPO Blue FreedomRx Plan. Unsure of when to enroll in Medicare, review the helpful chart: When to Enroll in Medicare. You must have Medicare A and B to enroll in these plans.
2024 Retiree Insurance Rates
These premium rates are the retiree monthly costs. This is 50% of the full premium. The Town of Hanover also contributes 50% of the premium for all retired employees and their spouses. Rates subject to change upon death of retiree.
- Medex 2 and Blue MedicareRx PDP is a total of $195 per person
- Medicare PPO Blue FreedomRx is a total of $171 per person

Enrollment Forms
If you are turning 65 and wish to enroll in either Medex 2/Blue MedicareRx PDP or Medicare PPO Blue FreedomRx through the Town of Hanover, complete the electronic enrollment form below. You will be required to submit your Medicare Part A and B card using the paperclip icon at the bottom of the form. If you are the spouse of a retiree newly enrolling in the plan, you will also be required to submit a copy of your marriage certificate. Once you click "finish" your form will automatically be submitted to Rachael O'Callaghan, Personnel Coordinator for review and processing.
